We do dozens of these each week for clients. We usually, instead of putting the Business Name as the one "being impacted" replace it with the Business Name of the one "being reported".

Also, the part that says to upload a file, that's not the images as proof. That's only for BULK reporting (10 or more). This is a common mistake we see others making.
